Instead of two cancerous parties, you have five cancerous parties, but at least there’s some vote migration and resulting shifts of power. In case of US it’s a literal duopoly.

It’s shitty but stable. Many people use private services for more mundane issues like dental ect. And while it’s shitty, it won’t bankrupt you. In some cases, homelessness in US is caused by health issues combined with Healthcare costs. Here it just doesn’t happen.
